Our memorial plaques are created to honor their service and sacrifice, and to provide a lasting tribute for the families and departments who carry them forward.

While our work began in California, our mission has always been larger than state lines. We believe every hero deserves to be recognized, no matter where they served. This page allows anyone to notify us of a fallen officer or K9 so we can ensure their legacy is honored.

Plaques are created exclusively for the families and departments of fallen officers and K9s.

Anyone may submit a request on their behalf, but plaques will only be shipped to verified law enforcement agencies, who ensure they are delivered to the appropriate family members.
